Patient Pre-Visit Check List

Thank you for your interest in Education and Evaluation Center for Medical Marijuana and exploring marijuana in providing you relief treatment for your medical condition.

Before you come in, please review the following check list and requirements. Please bring the necessary documentation with you to your appointment.

• You must be a legal resident of the State of California
• Bring a valid California Driver's License or California-issued identification card.
• If you are less than eighteen years old, you must be accompanied by your parent or legal guardian.
• Bring a copy of your pertinent medical records. Your current physician / hospital can explain their procedure to obtain these records.
• Bring in all medications you are taking, or a list of all medications.
• You must not currently be on legal probation.
